Hello! Thank you for your product change request. 

 

With recent changes from the TGA, Heyday has updated the way that we care for our patients in order to remain compliant. Product change request forms are no longer available and will require an appointment with a doctor. We understand how this change is frustrating and inconvenient.

 

We continue to maintain a gold-standard level of care & patient experience; and will accommodate your needs with the new changes as necessary.
